Do you provide your own playing strip?
If you are a coach of a team that does not use the playing shirts provided by the PJSA you may claim $10.00 refund per player.
Follow these steps:
- Download and print out the Payment Request Form.
- Fill in the form.
- Attach a list of your registered players.
- Submit to the Field Manager at Bortollo (Bob Pond) or Singleton (Tony Gillespie). Alternatively post to the PJSA.
- The nominated payee will receive a cheque in the mail.
NOTE: If you are making a claim on behalf of several teams you must fill out a form for each team. If teams nominate a common payee then only one cheque will be issued.
